We have provide the forms necessary to create records inventories for your department or send records for disposition to either the State Records Center or the University Archives. More information on forms can be found in the Records Management Manual.

If you should have any questions regarding the use or submission of information in any of these forms, please contact the University Records Officer.


RECORDS INVENTORIES
The records inventory forms were designed by ARMS to assist you in conducting an inventory of your records. Although these inventories may be helpful in writing Record Retention/Disposition Authorizations (RDAs), the use of these is not required (you may simply use the RDA Form listed below). Please see the Records Management Manual for help preparing the inventory forms.

  • Form UWA-4: Records Inventory Worksheet (MS Word)
    Use this worksheet to create an inventory of your department's records.
  • Records Inventory Worksheet (MS Excel)
    Use this worksheet to create an inventory of your department's records.
  • Form UWA-5: Electronic Information Resources Inventory (MS Word)
    Use this form to inventory your electronic records and recordkeeping.

CREATING A RECORDS RETENTION/DISPOSITION AUTHORIZATION (RDA)
This interactive form has been create so that you may begin the RDA creation process. The University Records Officer will review your application and contact you for further information. Information on this form can be found in the Records Management Manual. The Inventory Forms (see below) may be helpful as preliminary working documents.

  • Records Destruction/Disposition Authorization (RDA) Form (MS Word)
    Use this to submit an application for creating a new RDA. Once completed, email to recmgmt@library.wisc.edu.
    The ARMS office will review the form and contact you regarding your application.

SENDING RECORDS TO THE STATE RECORDS CENTER
PLEASE NOTE: As of July 1, 2007, the State Records Center no longer will accept the transmittal forms. Departments must use the online records inventory system called Versatile. For more information, please visit Using the State Records Center.

SENDING RECORDS TO THE UNIVERSITY ARCHIVES
Use the following form to inventory your records you want sent to the Archives. Once you send an inventory, the ARMS office will review the form and discuss your needs.

  • Form UWA-1: Records Transmittal and Inventory (PDF)
    Use this form to inventory all of your records you wish to send to the University Archives. Instructions for completing the form can be found on with the information on Transferring Records to the Archives.

Please Note: This is not the form used to transfer records to the State Records Center. For information on the differences between the University Archives and the State Records Center, please visit State Records Center vs. UW Archives.
