Forms and Boxes
The following information details what forms University departments should use when either creating an inventory of records, filling out the RDA form, the new procedure for sending boxes to the State Records Center, and sending records to the UW Archives. It also provides information regarding purchasing the boxes to house records (the State Records Center Cartons). Please visit our Downloadable Forms page for any of the forms listed below.
Inventory Forms
The Information Resources Inventory form (UWA-4) and the Electronic Information Resources Inventory form (UWA-5) were designed by ARMS to assist you in conducting an inventory of your records. Although they are may be helpful in writing RDAs, the use of these is not required. Instructions for completing an Information Resources Inventory form are found in the Records Management Manual.
RDA Form
See the Forms page to download the RDA form used by UW-Madison, or call 262-5629 to request a form. Information on completing these forms is found in the Records Management Manual.
The Inventory Forms (see above) may be helpful as preliminary working documents. If you would like to submit an RDA electronically, please send an email to recmgmt@library.wisc.edu or call 265-9287.

Transferring Records to the State Records Center
The procedure to transfer records to the State Records Center has changed. The State Records Center no longer will accept the transmittal forms. Departments must use the online records inventory system called Versatile. For more information, please visit Using the State Records Center.

UW Archives Transmittal Form
Use the Records Transmittal and Inventory Form UWA-1. See the Forms page to download the form or call the Archives at 262-5629 to request a copy. Instructions for completing the form can be found on with the information on Transferring Records to the Archives.

Boxes: Records Center Cartons
Records must be packed in records center cartons. The cartons are available through Materials Distribution Services (MDS). The records center carton will accommodate both letter and legal size files. If your records require the use of odd-size containers, contact the appropriate storage facility as soon as possible to discuss boxing and transfer arrangements.

State Records Center Price List
This is available online: State Records Center prices. Storage costs are covered via the University and there is no outward cost to departments for using the State Records Center.
